Bias Incident refers to verbal or physical action committed against or directed toward a person or to a person’s property because of the person’s actual or perceived race, color, creed, religion, national or ethnic origin, ancestry, age, sex, gender identity or expression, marital status, order of protection status, disability, military status, sexual orientation, pregnancy, or unfavorable discharge from military service.
If you would like to report a general incident that does not fall into one of the other reporting categories, please use this form.
Share significant concerns for a student’s well-being (social, financial, family, mental health, etc.).
Use this form to report incidents of sex discrimination, including sexual harassment, sexual assault, sexual exploitation, domestic violence, dating violence, and stalking. This form is for individuals seeking support and information after experiencing any form of sex discrimination or sex-based harassment, third party witnesses, anonymous reporters, and College employees fulfilling their responsible reporting duty to share information disclosed to them. This form does not initiate a formal investigation, but will prompt a response from the Title IX Coordinator.
