To help us provide the best care possible when you are on campus, it's important for us to have up-to-date medical information about you. When you arrived on campus as an incoming student, you filled out a Emergency Medical History Information Form (PDF) and Immunization Form (PDF). Please resubmit these forms if there have been updates since those forms were last submitted to the College.
E-mail the forms to Becky Canfield; fax them to 309-341-7077; mail them to the Division of Student Development, Knox College, Box K-236, 2 East South Street, Galesburg, IL 61401; or drop them by the Division of Student Development.
Note to incoming students: We must have your Emergency Medical Information Form and Immunization Form before you can register for classes. Visit your personalized student portal to complete these forms.
If you need to have vaccinations while on campus, contact the Knox County Health Department.
If you have any other questions regarding immunization requirements or the Emergency Medical History Information Form, call the Division of Student Development at 309-341-7222.
Athletes at Knox College will not be allowed to participate in practice, competitions, or organized off-season training sessions without a yearly physical examination. See when physicals will be offered on campus and learn about insurance and other forms required to be a Prairie Fire athlete.
Knox College and the Sports Medicine/Athletics Training program also provides many resources to handle injuries and/or illnesses suffered by varsity athletes.
