Bias Incident refers to verbal or physical action committed against or directed toward a person or to a person’s property because of the person’s actual or perceived race, color, creed, religion, national or ethnic origin, ancestry, age, sex, gender identity or expression, marital status, order of protection status, disability, military status, sexual orientation, pregnancy, or unfavorable discharge from military service.

Title IX, Sexual Misconduct, and Gender-Based Violence Report

Use this form to report incidents of sexual misconduct, including sexual harassment, sexual assault, domestic violence, dating violence, and stalking. This form is for individuals seeking support and information after experiencing sexual misconduct, third party witnesses, anonymous reporters and College employees fulfilling their responsible reporting duty to share information disclosed to them. This form does not initiate a formal investigation, but will prompt a response from the Title IX Coordinator.
